In order to make your first appointment more pleasant, you can fill out and print online our Patient Demographics Questionnaire and our Patient History Questionnaire.

Please bring with you or have your physician send medical records that relate to your current condition. If you prefer, we will request those records for you prior to your visit. Contact us with names and addresses of your physicians.

Please arrive 15 minutes prior to your first scheduled appointment to complete new patient registration. We will need the names and dosages of medications you are currently taking, as well as your family's medical history and any allergies you may have. Your first visit will take about 1 to 1-1/2 hours.

If you have difficulty speaking, do not speak English, or are otherwise incapacitated, please have someone accompany you on this visit.
